16th Annual Walk-A-Thon Registration

Use your Manual Collection Sheet to record the details of all your sponsors. Please print clearly and always enter the name and amount.

You must bring the completed Pledge Sheet and all monies collected to the Walkathon. Even if you are unable to collect any pledges you must still return your pledge form(s). You may still participate in the walk for a minimum donation of $20.

*NEW - All donation receipts are now sent electronically. If the sponsor would like a tax receipt they must provide a valid email address AND full mailing address with postal code. Tax receipts will not be issued for amounts less than $50.

If for any reason you are unable to attend the Walkathon you can either hand deliver this package and all money collected to one of the Trish's Wish executive or members named on the "Contact Us" page of www.trishswish.ca or mail it to: Trish's Wish Foundation, 53 Howard Avenue, Lindsay Ontario, K9V 2W1. If mailing, please include a cheque for the total amount of cash, do not send cash.
